As professionals dedicated to aging life care management we offer knowledge, experience, and compassion to help clients and their families with the challenges of aging and disability. Our mission is to enhance quality of life for older adults and those with disabilities, to help them maintain their dignity and independence, and to reduce the stress of caring for the frail or disabled relative.


We are seeking a Social Worker or Gerontologist interested in joining the rewarding and expanding field of Aging Life Care Management. We are looking for a dedicated, independent, creative and experienced professional with excellent interpersonal skills and a background in aging and health care. CCM certification a plus. Experience in geriatrics, dementia, end of life care and/or mental health also helpful. The aging life care manager position is a minimum of 20 hours per week up to 40 hours per week. You will need to be a self-starter, able to work independently, visit clients and communicate well with clients, families and other professionals. Pay is $25 – $40 per hour commensurate with experience.


The candidate would be available to visit our clients residing primarily in Montgomery County and northwest D.C. Our professional team consists of twelve aging life care managers (nurses, gerontologists and social workers) who work well together and support each other. We have a dedicated office staff that supports the practice and aging life care management team.

Qualifications:


1.         MSW, LCSW, CCM or MS in Gerontology required


2.         Active license in MD and Washington, D.C., where applicable


3.         Good verbal and written communication skills


4.         Driver’s license
